1:1 Support Services (In-home & Community)

We understand that each client and their circumstances are unique. That's why we take a holistic co-design approach, including working with our clients and their families to create support plans and activities that meet their specific needs.

Within our 1:1 in home and community access supports, we assist our clients to transform into robust individuals with a sense of confidence and increased independence.

Some of the support we provide 1:1 to our clients in home and in the community:

  • Self-care tasks

  • Community access

  • Meal preparation

  • Building independent living skills

  • Building daily routines

  • Introducing and facilitating self discipline

  • Building self-confidence and self-love

Our 1:1 approach to Support Services not only includes typical activities crucial to building our clients independent living skills, but also a holistic approach to betterment, by incorporating mindfulness and well-being through:

  • Yoga

  • Meditation

  • Outings in nature

  • Physical activity

Along their journey, each participant is supported and empowered to take a step out of their comfort zone in order to become the best and healthiest version of themself.

Psychosocial Recovery Coaching

Physical activity and exercise is part of our wellbeing philosophy at Abilitive and forms a core part of our holistic approach to rehabilitation. We offer Psychosocial Recovery Coaching as part of our service offering and not only look at an individual’s injuries and symptoms but their entire system as a whole.

Our team are trained in psychosocial recovery coaching and work closely with individuals, their families and broader supports to develop and implement programs and activities as part of recovery plans. This method of co-design allows for individuals to feel supported, empowered and to take the steps required to regain control of their life.

Our process in developing the best recovery plan for individuals, includes:

  • Understanding the client and their needs

  • Determining their comfort levels and current abilities

  • Collaborating with their families and broader supports

  • Introducing the right activities and motions, with close supervision

  • Enabling individuals to discover new strengths and abilities

The work we have done with our clients has shown that with the right activities, they have been able to change their outlook in life, build capacity and develop new skills that help them regain control of their life.

We have a firm belief that with better physical health comes better mental health and a newfound appreciation of each day.
